Low sperm count treatment begins with a question most reports do not answer. Why is the count low? Many men who walk in with a poor result have something correctable sitting behind it. Others will need ICSI. The two paths look nothing alike, and treating the number instead of the cause is how people lose six months.
The WHO reference limits
The WHO 2021 reference values set the lower limits from the fifth centile of fertile men. Below 16 million per millilitre, or a total below 39 million per ejaculate, is oligospermia.
Why the number alone decides nothing
Concentration is one figure on a report full of them. Total motile count after processing, motility and morphology all pull on the outcome. So a man at 12 million with excellent motility sits in a better position than one at 18 million with poor motility. Same report, opposite conclusions.

Retrieval or donor discussion, after cause is established |
Structural and physical causes
Varicocele, which is dilated veins around the testis, raises the local temperature and depresses production, and of the structural causes it is the one most often corrected. Other things leave their mark too. An undescended testicle in childhood, a hernia repair, mumps orchitis, epididymitis.
Hormonal and genetic causes
Three hormonal patterns suppress testicular function, and all three are treatable once named:
Genetic causes sit in a different category. Klinefelter syndrome and Y chromosome microdeletions show up on karyotype, and they matter because they change what is possible rather than what is prescribed. The AUA guideline on male infertility sets out when to test.
Lifestyle and medication
Smoking, obesity, heat exposure and anabolic steroids all depress parameters, and so does testosterone replacement, which surprises most men who hear it, because rather than supporting your own production it shuts it down. Chemotherapy does the same. So do some antibiotics.
Our post on understanding sperm DNA tests covers the extra layer worth adding when a standard analysis sits on the borderline.
Medical treatment
Where a hormonal cause is confirmed, targeted treatment often works, and gonadotropin injections will restore production in hypogonadotrophic hypogonadism. Clomiphene gets used off-label too. Outside a proven deficiency the evidence for it thins out, so ask why it is on your prescription.
Surgery
Varicocele repair improves parameters in the right men, and a 2025 meta-analysis of multivariable studies went looking specifically at which men those are, which is the question that decides whether surgery helps you. Worth considering before IVF where the varicocele is clinically significant.
Assisted reproduction
Where correction is not possible, ICSI treatment needs one viable sperm per egg, so the numbers problem disappears. With azoospermia, retrieval through TESA, PESA or micro-TESE collects sperm directly from the testis or epididymis instead. And where none is found at all, our guide on donor sperm decisions sets out how that conversation goes here.
Can a low count improve on its own?
It can, where the cause is lifestyle, a hormonal imbalance or a treated infection. Sperm take roughly 74 days to mature. Give any change two to three months before you re-test.
How many samples do I need?
Two. A single abnormal result is not a basis for treatment decisions, so repeat it four to eight weeks later after two to five days of abstinence.
Does a low count rule out natural pregnancy?
Not with mild oligospermia and good motility. Below 5 million per millilitre the odds drop sharply and a structured plan serves you better than waiting.
Low sperm count treatment that starts with the cause almost always beats one that starts with the count. It is recommended that you ask what is driving your result before you accept any plan built around it.
