IUI Success Rate: What the Numbers Depend On

IUI Success Rate: Age, Timing and Key Factors

By Dr. Ram Prakash, Maaeri Fertility & IVF Centre.

The IUI success rate is one of the first things couples ask. It is also the hardest to answer in a single figure. The number moves more with who is being treated than with the procedure, so a percentage drawn from thousands of cycles says little about your own. Averages are built out of people nothing like you.

What Is the Average IUI Success Rate Per Cycle?

The ranges generally reported

Three figures get quoted at you:

  1. Around 5% for an unstimulated cycle
  2. Roughly 10 to 15% on oral medication
  3. Up to 20% with gonadotropin stimulation

Orientation, then. Not prediction. The spread is that wide because the populations behind each figure differ enormously, and nobody quoting a range says which one you resemble.

Why per cycle is the wrong unit

IUI is planned as a series. One attempt is not the unit of measurement, so the question that matters is what three or four cycles together are likely to deliver. The HFEA notes that IUI succeeds less often per cycle than IVF and that several attempts are commonly needed.

Protocol

What it changes

Trade-off

Natural, no medication

Nothing. Relies on your own ovulation

Lowest rate, suits regular young cycles

Letrozole or clomiphene

Makes ovulation predictable and timeable

Modest gain, oral and low cost

Gonadotropin injections

Recruits more follicles

Best rates, with a real multiple-pregnancy risk

Which Factors Influence It Most?

Sperm quality after washing

Post-wash total motile count is the strongest male-side predictor. A synthesis of two decades of IUI research puts the threshold for better outcomes at 5 to 10 million or above. Below that the odds fall away and the IUI versus ICSI conversation becomes the useful one.

The uterine side

The same review names two more predictors. Endometrial thickness of 6 to 10 mm. The number of mature follicles. Both show up on your monitoring scan. Both can be influenced by the protocol. So ask.

Age and diagnosis

Age is the dominant variable. No protocol fixes it. IUI also needs at least one open tube, which rules it out for bilateral tubal blockage and severe male factor. Our guide on female fertility tests covers how patency and reserve get assessed first.

How Many Cycles Are Worth Attempting?

Where the returns flatten

Three to six cycles. That is the usual window for a suitable candidate, and past it the per-cycle probability stops improving. Carrying on without reassessing buys you time spent, not a better chance.

When IVF is the better arithmetic

This matters most where ovarian reserve is low. In young women with unexplained infertility and diminished reserve, one comparison found live birth of 22.6% with IVF against 11.3% cumulatively across IUI. That is a specific group, not everyone. In that group the case for moving sooner is strong.

How Can You Improve Your Odds?

What is actually in your control

Age and diagnosis set the ceiling. Nothing on the list below raises it. Everything below works inside it.

The five that are worth doing

  • Use a stimulated cycle where it is appropriate: medicated cycles outperform natural ones for most candidates
  • Time it on a scan, not a kit: IUI should be coordinated with a trigger and confirmed follicle tracking. Our post on fertile days explains why
  • Give sperm parameters 60 to 90 days: sleep, smoking and alcohol changes show up in a repeat analysis, not immediately
  • Ask about luteal support: progesterone after a stimulated cycle adds little cost
  • Treat thyroid and PCOS first: both affect ovulation and implantation, so fixing them changes the baseline

Frequently Asked Questions

Does IUI hurt?

Brief, and comparable to a smear for most women. Mild cramping at worst, if that.

Can IUI work with low AMH?

It can, if a tube is open and sperm parameters hold up. Low AMH affects how many eggs a stimulated cycle yields more than it affects IUI mechanics.

What if three cycles fail?

Review before you repeat. If nothing has changed in the semen, the ovulation data or the protocol, a fourth identical cycle is unlikely to behave differently.

Wrapping Up

So read the IUI success rate against your own thresholds rather than the average. That is the difference between a cycle that teaches you something and one that just passes. The detail on IUI treatment sets out what a planned series involves here.
